Presentation

A 28-year-old breastfeeding woman, currently 6 weeks postpartum with her first child:

Therapeutic management

For the mother

1st-line local treatment
  • permethrin 5% cream
    • Apply a thin layer over the entire body, excluding the face, in the evening. Leave on overnight (minimum 8 hours) and rinse off in the morning. Repeat application between D8 and D14.
  • or
  • benzyl benzoate 10% cutaneous emulsion**
    • Apply over the entire body, excluding the face. Leave on for 24 hours then rinse off. Repeat between D8 and D14.
  • or
  • ivermectin at 0.2 mg/kg (2 doses: D0 and between D8 and D14, mid meal)
    • Off-label, recommended where skin condition is poor, especially if nipples affected
Breastfeeding management
Pump milk as long as skin lesions are present on the chest and as long as local treatment continues.
Related environmental measures
Decontamination of bedding and clothing by machine washing at 50°C or 3 days in a sealed bag, treatment of mattress, furniture and car seats with anti-parasitic spray.

For other family members

1st-line local treatment of the father
  • ivermectin at 0.2 mg/kg (2 doses: D0 and between D8 and D14, mid meal)
1st-line local treatment of the 6-week-old newborn
  • treatment is not advised in the absence of clinical signs of scabies
  • if clinical signs or itching behaviour are present, permethrin 5% cream is recommended
    • Apply a walnut-sized amount of cream in the evening over the entire body, excluding the face. Leave on overnight (minimum 8 hours) and rinse off in the morning. Repeat application between D8 and D14.
Related environmental measures
